Upstream is a clinical workflow tool used by your specialist to build and deliver a personalised Metabolic Health Plan. It is provided exclusively to patients of the clinic for that purpose.

The Metabolic Health Plan generated by Upstream represents your specialist's clinical judgment, captured and delivered through this tool. Upstream itself does not provide medical advice. Always discuss changes to medications, training, nutrition, or any other aspect of your plan with a qualified healthcare provider.

Accuracy of intake responses

By submitting the intake form you confirm that the information you provide is accurate to the best of your knowledge. Inaccurate information may lead to a plan that is unsuitable for you.

Use of the patient portal

The portal link issued to you is for your personal use only. Do not share it. If your link is compromised, contact the clinic and we will revoke and reissue.

Limitation of liability

To the extent permitted by law, the clinic and the operators of Upstream are not liable for any indirect, incidental, or consequential loss arising from your use of the service.
